Incident Investigation & Root Cause Analysis (RCA)

Risk & Process Safety Header

What is a Incident Investigation & Root Cause Analysis (RCA)?

Incident Investigation is a structured process for examining workplace accidents, near misses, dangerous occurrences, and occupational illnesses — to understand what happened, why it happened, and what needs to change to prevent recurrence. Root Cause Analysis goes beyond immediate causes to identify the underlying systemic, organisational, and behavioural factors that contributed to the incident. Techniques such as Fault Tree Analysis, Fishbone Diagrams, 5-Why Analysis, and Bow-Tie methodology are used depending on the complexity and severity of the event.

The output is a documented investigation report with findings, root causes, and a corrective action plan — providing management with the information needed to address systemic gaps and demonstrate due diligence to regulators and stakeholders.
